You must have one of the following qualifying conditions to be eligible for a medical cannabis card. *Any type or form of cancer *Chronic or severe pain *Epilepsy or other intractable seizure disorder *Multiple sclerosis, muscle spasms, or spasticity *Chronic nausea or cyclical vomiting syndrome *Post-traumatic stress disorder 2. You must be a Kentucky resident with a valid Kentucky ID. 3. You can not be convicted of a disqualifying felony offense. Prior to applying to the Kentucky medical cannabis program, you must obtain a medical cannabis certificate from a board certified provider. This certificate confirms that the patient meets program criteria and has a qualifying medical condition that medical cannabis is recommended as part of the patient's treatment plan. To start the process you need to schedule your initial in person visit with Danielle Guminski, a licensed medical cannabis provider. Danielle will review your medical history, educate you on the benefits and risks of medical cannabis, and provide a written and electronic medical cannabis certificate.
